Lonely Seaside Dentist Inc provides general, preventive, restorative, and cosmetic dental services. All services are provided by licensed dental professionals in accordance with California state regulations and professional standards of care.

Appointments

• Appointments can be scheduled online, by phone, or in person.
• We require at least 24 hours notice for cancellations or rescheduling.
• Repeated no-shows or late cancellations may result in a cancellation fee of up to $50.
• We reserve the right to reschedule appointments due to emergencies or unforeseen circumstances.
• Please arrive 15 minutes early for new patient appointments to complete necessary paperwork.

Payment Terms

• Payment is due at the time of service unless other arrangements have been made.
• We accept cash, checks, credit cards (Visa, MasterCard, American Express, Discover), and CareCredit.
• For patients with insurance, we will file claims on your behalf, but you are responsible for any amounts not covered.
• Estimates provided are based on information available and may differ from final charges.
• Outstanding balances over 90 days may be subject to collection procedures.

Insurance

We accept most major dental insurance plans. However, we are not responsible for what your insurance covers or does not cover. You are responsible for understanding your own insurance benefits. Our team will assist in verifying benefits and filing claims, but final payment responsibility lies with the patient.

Treatment Consent

Before any treatment, we will explain the proposed procedure, alternatives, risks, and expected outcomes. By consenting to treatment, you acknowledge that you understand these factors and agree to proceed. You have the right to refuse or discontinue treatment at any time.

Medical History

You are responsible for providing accurate and complete information about your medical history, current medications, allergies, and any changes to your health. Failure to disclose relevant information may affect the safety and effectiveness of your treatment.

Limitation of Liability

While we strive to provide the highest quality dental care, we cannot guarantee specific outcomes. Dental procedures carry inherent risks that will be discussed with you before treatment. We are not liable for complications that arise despite proper care and technique.
